It is important that you complete the form in full and ensure that the correct payment is sent otherwise the form will be returned and your affiliation will be delayed.

Club Status
If you are unsure as to what status your club should be affiliated under please check with the league the club is entering or alternatively contact the county office at info@kentfa.com.
Insurance
We are pleased to have mandatory Public Liability Insurance on a per CLUB basis and Personal Accident Insurance for players on a per TEAM basis. We now also offer player-to-player Public Liabilty Insurance. For further information on this please contact the office at info@kentfa.com.
Affiliation number
Your affiliation number will be sent to you once you have successfully submitted your Affiliation Form in a complete and correct order. The affiliation number will be found on the first line of payments under the heading affiliation and will begin with the letter H. for example HJD1234567.
Closing date for affiliations & cups
The closing date for affiliations and Cup Entries is 1 July 2010.
